Say With Sarah is a mental health practice specializing in treating anxiety disorders (generalized, panic, phobias) and Obsessive Compulsive Disorder (OCD). Sarah Weber Belka, the founder of Say With Sarah, has worked in higher levels of care for OCD treatment and has extensive experience providing evidence-based treatment for individuals with anxiety disorder and OCD.

Say With Sarah Anxiety & OCD Intensive Treatment Program provides a high quality treatment program composed of evidence based practices. Sarah uses a variety of therapies such as ACT (Acceptance & Commitment Therapy), CBT (Cognitive Behavioral Therapy), ERP (Exposure and Response Prevention), Inference Based CBT (I-CBT), and self-compassion.

The standard of care in the Say With Sarah Anxiety & OCD Intensive Treatment Program is high— there are no groups, no solo time trying to figure out treatment by yourself, and no behavioral technicians.

The program is designed to provide a high degree of therapist involvement through individualized treatment planning, live coaching, skills practice, and direct clinical support throughout scheduled treatment hours.

The Say With Sarah Anxiety & OCD Intensive Treatment Program consists of a three-week intensive treatment experience with Sarah Weber Belka. Each program comes with OCD assessment, a three hour in-person or telehealth counseling session per day, between-session coaching and support (maximum 1 hour per day), and provides access to a private electronic drive for therapy homework and resources.

The Say With Sarah Anxiety & OCD Intensive Treatment Program is a structured, short-term psychotherapy program designed for adults experiencing symptoms of Obsessive Compulsive Disorder (OCD) and anxiety disorders. The program provides individualized treatment using evidence-based interventions, including Exposure and Response Prevention (ERP), Inference-Based Cognitive Behavioral Therapy (I-CBT), Acceptance and Commitment Therapy (ACT), Cognitive Behavioral Therapy (CBT), and related approaches when clinically appropriate. The standard program consists of three weeks of treatment, four days per week, with three-hour treatment sessions scheduled Monday through Thursday. Treatment may be provided in person or via telehealth, depending on clinical appropriateness and licensing requirements.

Services include:

  • Initial assessment and treatment planning

  • Individualized ERP or I-CBT treatment

  • 3 hour treatment sessions per day, Monday-Thursday 9am-12pm PST, 36 hours of total treatment.

  • Between-session coaching and support, limited to a maximum or one hour per day, excluding crisis.

  • Treatment materials, worksheets and assigned therapeutic exercises.

  • Outcome measurement and progress monitoring.

  • Relapse prevention planning and discharge recommendations.

Appropriateness For Services: Admission to the program is based on clinical assessment and professional judgment regarding appropriateness of care. This program may be appropriate for individuals who:

  • Are age 18 or older

  • Reside in a state where Sarah Weber Belka is authorized to practice (Oregon and Washington)

  • Are experiencing mild to moderate levels of OCD or anxiety symptoms which are interfering with daily functioning, approximately 3-8 hours per day maximum (evaluated upon consultation).

  • Have not achieved desired progress through traditional outpatient therapy alone

  • Are able and willing to actively participate in intensive treatment

  • Are willing to complete between-session therapeutic practice assignments

Program Pricing & Financial Agreement:

Total Program Cost: $12,480

The Say With Sarah Intensive Program is a comprehensive three-week treatment program rather than a collection of individual therapy sessions. Program fees reflect the reservation of dedicated treatment time, assessment, treatment planning, direct clinical services, between-session support, therapeutic materials, progress monitoring, care coordination, and relapse prevention planning.

A $2,400 deposit is required no later than 48 hours prior to the start of the program to reserve treatment time. The deposit is applied toward the total program cost and is not an additional fee. Because intensive treatment slots are reserved exclusively for each participant and generally cannot be reassigned on short notice, the deposit is non-refundable once the reservation has been confirmed.

The remaining balance is paid according to the following schedule:

• Week 1: $3,500
• Week 2: $3,500
• Week 3: $3,080

Clients are charged at the beginning of each treatment week. Program fees are based on reserving a dedicated treatment schedule and are not calculated on an hourly or daily attendance basis. Fees are generally not prorated, refunded, or reduced for missed sessions, late arrivals, voluntary withdrawal, early termination, scheduling conflicts, travel, illness, failure to complete assigned work, or failure to complete the program. Exceptions may be considered at the sole discretion of the practice in extraordinary circumstances.

The Say With Sarah Intensive Program is a private-pay service and is not billed directly to insurance. Clients may request a superbill for out-of-network reimbursement. Because insurance plans vary significantly in their coverage of intensive psychotherapy services, the superbill provided may not reflect the full duration or structure of the program. A daily superbill will include one psychotherapy service using CPT code 90837. Insurance companies often limit reimbursement for multiple psychotherapy services provided by the same clinician on the same day and may not recognize intensive treatment formats in the same manner as traditional outpatient psychotherapy.

Participation in the program and payment of program fees are not contingent upon insurance reimbursement. Clients are responsible for all program fees regardless of whether reimbursement is approved, denied, reduced, delayed, or otherwise limited by their insurance carrier. Submission of a superbill does not guarantee reimbursement.

Insurance & Reimbursement: The Say With Sarah Anxiety & OCD Intensive Treatment Program is a private-pay service and is not billed directly to insurance. Clients who wish to seek out-of-network reimbursement may request a superbill.

Because insurance plans vary significantly in their coverage of intensive psychotherapy services, the superbill provided may not reflect the full duration or structure of the program. The program is structured as an intensive outpatient service consisting of multiple therapeutic activities delivered over a three-hour treatment block, reimbursement through out-of-network benefits may differ from traditional weekly psychotherapy. Insurance companies vary widely in how they evaluate, process, and reimburse claims for intensive services.

A daily superbill will include one psychotherapy service using CPT code 90837. Additional treatment hours provided as part of the treatment program are not separately submitted on the superbill. Insurance companies often limit reimbursement for multiple psychotherapy services provided by the same clinician on the same day and may not recognize intensive treatment formats in the same manner as traditional outpatient psychotherapy. Participation in the program and payment of program fees are not contingent upon insurance reimbursement. Clients are responsible for all program fees regardless of whether reimbursement is approved, denied, reduced, or otherwise limited by their insurance carrier.

Program curriculum

  • Curriculum:The Say With Sarah Anxiety & OCD Intensive Treatment Program isn’t just about breaking cycles, it’s about restoring your connection to who you are outside of OCD! Sarah is a highly specialized therapist and is trained in multiple modalities for OCD. Together you’ll decide if your program is best suited for ERP or I-CBT. You can expect quality care with Sarah than a larger mental health company because of the 1:1 time and level of expertise. One unique feature of this intensive program is the absence of group therapy work. While group therapy can be highly effective, getting the attentive care you need to recover from OCD is important! Group is not required in this program, but if it is desired by participants there is an optional OCD support group offered at an additional cost ($60/meeting).

    The two evidence-based therapies for treating OCD are: Exposure & Response Prevention (ERP) & Inference-Based CBT (I-CBT). Exposure and response prevention therapy is considered the gold standard of treatment for OCD. It involves being exposed to stimuli or triggers in order to practice habituation and inhibitory learning with distress. Inference based CBT is considered an upstream cognitive model that identifies the faulty reasoning process that occurs with OCD doubts. Sarah will schedule an initial complimentary consultation for clients to make an informed choice on which therapy best meets their needs and go over expectations for treatment. Whichever modality is chosen, there is a requirement of between sessions work every day in order to achieve the maximum gains of the program!
